Jittery playback and sound is out of sync

Dawn of the planet of the apes has jittery playback and the sound is out sync starting with chapter 14. Even when I play the movie on my PC.
 
same thing

I have been having issues all along with jitter playback and out of sync sound
have been using both anydvdhd and clonebd to make isos

when writing the isos to bluray the playback is fine
but when making them into mp4 they are not,
this only happens for the first 5 minutes of playback
originally thought it was from using dlna server to watch mp4's
 
I tried to make a partial/movie only copy of 14 Blades, I got picture only, no sound.
