Hmm, there seems to be a mistake.
That disc has only one playlist (#0) representing the movie.
Super-simple disc.
What is the problem with it?
 
The movie comes out pixelated after extracting the files and merging it to mkv format. Audio and subtitles are fine.

That is not an AnyDVD problem or choice of playlist.
What did you use to make an MKV?

Please try CloneBD (download free version here), see if that works. It also has a built-in player - does that play pixelated video, too?
Don't "extract" files first - just use it right on the disc, please.
