Problems with corrupt directories

Lately a lot of movies I have tried to copy have failed due to corrupt directories. Is this something new that the industry has come up with or what?

Please answer these questions (especially 8 and 9)
 
Everythis is all in order to all of the questions. The problem comes during the copy stage about 50% through when the message "copy failed possible due to dirty disk or corrupt directory. Clean disk and retry". Naturally I cleaned the disk but to no avail. This doesn't happen often but it has happened several times lately and I thought perhaps the industry had come up with something new. If not, I suppose it simply disks that have been used too much and worn.

Yes, it sounds like bad source discs to me.
