Judging by the fact it died at around 50%, it's probably a layer change issue. If this drive you're reading it on is an LG model, try using one from a different manufacturer. I primarily use an LG, but LG's aren't perfect readers. Many times, I've come across discs it won't read at the layer change, but my Verbatim made by Pioneer will.
Agreed.Just speaking from general experience, I often run into discs that are brand new and pristine and they fail to read/rip. A replacement almost always solves that for me.
